Sustainable and contemporary health care models will see more people receiving care in alternative care settings. In recognition of this need, Queensland Health is continuing the $67.5 million Connected Community Pathways funding program for 2023/24. This funding program is focused on models of care that support or provide community-based care through partnership arrangements which deliver care closer to home and in a more coordinated manner. As per 2022/23, the 2023/24 CCP funding program will prioritise models of care that support the achievement of health equity with First Nations Queenslanders.

The Healthcare Purchasing and System Performance Division, Queensland Department of Health is offering two consumer representatives an exciting opportunity to join the Connected Community Pathways (CCP) Assessment Panel to review funding applications from Queensland Hospital and Health Services and Divisions within the Department of Health (DoH).

By contributing your lived experience and understanding of the health system to this strategic, high level panel, you’ll support the funding of models of care which strengthen access to care in the community and closer to home through networked models of care and community partnerships.
